Incident Report:
County or House coaches may evaluate any person they are required to deal with because they are a coach of a VYI basketball team. The results of the evaluation will be forwarded to the Commissioner of the league in question for further action as deemed appropriate by the Commissioner and league officials.

Teammate Request 2010:
This form is required before any commissioner will consider a request for a specific teammate and must be completed prior to November 1. Only ONE (1) name may be entered. If either the player name or the requested name has not registered and paid in full, the request will be removed from consideration. If both families do not make the same request a single request will not be considered.

Waiver Request 2010:
This form is used whenever a waiver of all or part of the fees for basketball is requested. Prior to filling out this form ythe parents must:

1) register online
2) both parents volunteer (If parents volunteer for Building Director, both must take the current test listed in the left hand menu under Building Director Test 2008), (If parent volunteers for coach they must fill out the House Coach Application and contact their league commisisoner who will verify same),or p

County Coach Certification 2010:
This form is required to be filled out by all County basketball coaches prior to the season beginning to ensure that the coaches abide by all the rules of VYI in the selection and rostering of County teams.

County Coach Eval.:
This form is to be filled out in its entirety by any parent of a County player who wishes to comment on the quality of coaching they are receiving during the current County basketball season.

County Parent Agreement 2010:
Each County parent must read the pledge in this form and by signing their initials agree to abide by all the provisions of the agreement during this season.
